To Be Eligible For Assistance

Assistance for Children’s Diapers 

Children are eligible to receive services up until the age of 2 (child’s 2nd birthday). We do not provide services to children age 2 or older unless medically necessary and with appropriate documentation.

Exceptions will be made for clients with medical conditions or disabilities. 

This does not pertain to those needing assistance with incontinence products.

Identification is required for Parent or Guardian, and Proof of Parenthood is required for each child such as birth certificate, health insurance card with birthdate listed, hospital discharge papers or Foster placement letter.
